Client Intake and Allergy Card

Companion to the lessons "Legal setup beyond the license", "On site: service and safety", and "Keep them coming back"

One card per client, captured on your phone before the first service. This is both a safety document and a legal one: the intake card plus the patch-test record is what demonstrates you met the standard of care if anything ever goes wrong.

1. Health Screen (ask at every visit: "anything new since we last talked?")

☐ No ☐ Yes

Yes, it delays the first booking by a day. It also prevents the reaction that ends the client relationship, triggers an insurance claim, or reaches the board. Reactions to hair dye are the classic beauty-industry liability case.

3. Service Record (the client card is the product; write down more than the formula)

Hair texture and density, the chair by the window, the dog's name, the daughter's wedding date: the next visit should start exactly where the last one ended. Being remembered is the moat.

4. Arrival Questions (before the kit opens, every visit)

"Where's the best light and an outlet?"
"Any allergies or product sensitivities I should know about, anything new since we last talked?"

New matters: people develop dye sensitivities between visits. Setup spot: kitchen or dining chair by a window for cuts and color, hard flooring over carpet for nails, outlet check before the blow dryer comes out. You borrow only light, water, and electricity.

5. Recall Preference (if a client declines the prebook)
